Email Consent Notice

I hereby consent to sending the information (including my Advance Directive) I asked you to send on this screen via “unencrypted” e-mail. The information will be sent in plain text and can be read by anyone with access to the email. It is possible for someone besides the intended recipient to view the email and any information it contains, for example, if the email is hacked or otherwise unlawfully accessed. No matter whether the e-mail is encrypted or not, you must trust the recipient with the information. We will not be able to send the information if you refuse to send it via “unencrypted” e-mail. If that is the case, you can send the information yourself in any way you deem appropriate.
